For years, female genital mutilation has meant the death of sexual pleasure and caused pain, suffering and even death.

That is no longer true for many cases, in which simple reconstructive surgery can restore pleasure and open the vagina for non-painful intercourse and childbirth.

Today, a handful of FGM survivors in Kenya are receiving free reconstructive surgery, rehabilitation and counselling by French organisation Clitoraid. Its mission is: “Restoring a sense of pleasure and dignity”.

It’s a first in Kenya. There’s clamour for it and surgeons are being trained.

A human right

“Sexual feeling is a right, recognised by the UN as a human right. It is one of the basic human senses,” Dr Marci Bowers tells the Star. She is a gynaecological and reconstructive surgeon from California.

She leads the team that includes Dr Adan Abdullahi, a reconstructive surgeon at Nairobi University, Kenyatta and Karen hospitals, and Dr Loise Kahoro, a reconstructive surgeon from Kenyatta Hospital.

Surgeries are performed at the Karen and Mama Lucy hospitals.

“So many people are unclear about what happens in FGM,” Dr Bowers said. They think the clitoris is very tiny. But we know from reconstructive work that the clitoris — now that we have mapped it — is more than 11cm in length.

Even in the severest cases of FGM, 95 per cent of the clitoris is still there, Bowers said. “We are able to clean it up, bring it through the skin, bring it to the surface and sew it there.

“This is the operation. It’s just that simple and complications are very minor. It’s an amazing surgery.” she said.

Given the great extent of the problem, the aim is to make surgery accessible to everyone who wants it.

“We did not want it to be an opportunity that can be accessed only by rich women who have had the cut,” Dr Adan says. “We didn’t want it to be an elitist surgical procedure, we wanted it to be across the board.”

In recovery, patients experience some pain during the first 24 hours, but it is controlled with medication. They are able to resume most activities fully within a week and can even have sex after four to six weeks.

“When we ask women why they want the surgery, there is a sexual component, but the most important reason is this: they want it because they feel something was taken from them,” Dr Bowers said.

“The surgery is an attempt to regain their identity and what was lost. To take charge of their body.”

Age should not be a deterrent, she said. The oldest patients treated by the organisation are in their 60s. The youngest undergoing surgery in Kenya is 18.

“There is no age at which you lose sexual feeling. We think of sex as a young person’s thing, but we don’t lose our sense of touch or smell as we age, so sexual feelings can be enjoyed at any age,” Bowers said.

“Life is to be enjoyed, and this is one of the basic senses.”

Controversy in Burkina Faso

However, in 2015, the organisation had problems in Burkina Faso. Doctors’ licences were revoked and plans for a clinic were cancelled by the government.

They have speculated this may have been caused by politics or money, since there’s money to be made in performing FGM. It is also speculated the ban could be linked to the religion of the founder of Clitoraid, Maitreya Rael, who started the religion Raelism. Followers believe the founder was visited by aliens, who explained human origins and how to plan the future.

It was suggested that ‘cut’ women in Burkina Faso, out of gratitude for reconstructive surgery, would abandon traditional religions and join the movement, a claim Clitoraid has denied.

“We only go where we are welcome. We realise there are skeptics and that is why it’s important to be objective and to put science behind what we do,” Dr Bowers said.

“It was very murky, very sinister. Why the opposition? Why would the government, so far removed from the actual people, object to this? It just doesn’t make sense,” Bowers said.

Dr Bowers maintains contact with all her patients who have her mobile number and email. “One of the things I love is getting awakened at 2am because someone had their first orgasm,” she said.

Alan Dershowitz Joins Legal Team Defending Female Genital Mutilation on Religious Freedom Grounds

High-priced defense attorney Alan Dershowitz has joined the legal team for three Michigan residents accused of conducting female genital mutilation on two young Minnesota girls.

This is the first such case prosecuted in the United States. The defense team is expected to make a two-pronged argument, saying that what happened to the girls does not qualify as “female genital mutilation” but was instead “legal and benign.” Moreover, they are expected to argue that the practice is constitutionally protected as a religious practice.

Dr. Jumana Nagarwala, along with Dr. Fakrhuddin Attar and his wife, Farida, belong to the Detroit-area Dawoodi Bohra community, a sect of Shiite Islam with more than a million adherents worldwide.

The Dawoodi Bohra’s religious leader, Syedna Muffadal Saifuddin, endorsed female genital mutilation at a Mumbai mosque last year, saying it “needs to be done.”

Dershowitz, whose past clients include Jeffrey Epstein, Jack Kevorkian and O.J. Simpson, was retained by the Dawat-e-Hadiyah, an international organization that serves as the administrative arm for the Dawoodi Bohra community, the Detroit News reported.

The trial will begin on Oct. 10 in a Detroit federal court. The two doctors face up to life in prison if they’re convicted, while Farida Attar, Fakhruddin’s wife, could serve 20 years. All three are currently being held without bond after it was determined they are a flight risk.

A central legal question will be whether the procedure—also known as “khatna” or “female circumcision”—harmed the two seven-year-old Minnesota girls.

Because male circumcision is not deemed harmful and can have health benefits, it is not unlawful. In contrast, courts have historically ruled against parents deemed to have caused their children harm when adhering to a religious practice, such as Jehovah’s Witnesses who denied their children medical care.

Shannon Smith, a defense attorney for Nagarwala, has said that instead of mutilating the girl’s genitals, her client used a “scraper” to wipe off a portion of the mucus membrane around their clitorises. The practice did not cause harm and had cultural and religious significance, she said.

But the World Health Organization said the practice has no health benefits. And according to the 11-page complaint filed against Nagarwala, one of the seven-year-old girls described screaming, feeling “pain all the way down to her ankle” and scarcely being able to walk after the procedure.

At a May hearing, U.S. Magistrate Elizabeth Stafford denied bond to Fakhruddin and Farida Attar, calling their religious defense “a shield,” the Detroit Free Press reported.

“It is important to me … to take religion out of it and focus on the allegations that young girls’ genitals were mutilated and that the defendants played a role. … I think it’s common knowledge that the cutting of the genitalia of a 7-year-old child would be painful. I find this to be a serious crime,” Stafford said.

A New Way to Prevent Awareness Under Anesthesia

Slow brain waves reveal precisely when a patient loses awareness while under anesthesia, and could prevent the small percentage of cases in which patients are "awake" during surgery.

Being aware of what's happening during surgery and even feeling the pain seems like an unthinkable nightmare. Isn't that what anesthesia is for?

But it does happen in up to one percent of surgeries involving high-risk patients, according to research published in 2011, and affects between 20,000 and 40,000 patients annually in the U.S. alone. Now, scientists from the University of Oxford in the U.K. believe they've found a way to put an end to this disturbing statistic.

Using EEG brain monitoring and MRI imaging scans, the researchers discovered that people lost awareness when low-frequency electrical waves, also called “slow waves,” enveloped the brain. When the waves reached a plateau, sensory signals no longer reached the thalamocortical regions, which are the parts of the brain linked to conscious awareness.

“Awareness in anesthesia is a 'never event'—it isn't good enough for it to be rare,” Roisin Ní Mhuircheartaigh, one of the researchers, told Healthline. “Our goal is to allow anesthesiologists to look at a patient's brain activity and know with confidence that [he or she] is safely asleep.”

The researchers have applied for a patent on their findings and are looking into developing better monitoring equipment for patients under anesthesia. They are the second group of scientists this year to do so. Earlier this year, researchers from the Massachusetts Institute of Technology and Boston University published their findings on slow waves and unconsciousness.

“They looked at EEG, too, but have focused on the relationship between slow waves and alpha activity,” Catherine Warnaby, another Oxford researcher, told Healthline. “A key difference is that we have looked at slow wave saturation and have the FMRI evidence to support that this state represents a state of perception loss.” Changing the Standards for Anesthesia

Warnaby stressed that anesthesia is very safe, but little is known about how it works in the brain. In patients with severe health problems, too much anesthesia can adversely affect their heart or lungs. Elderly patients may experience severe confusion after an operation if given too much anesthesia.

“We think that this has great potential to become an individualized marker for delivering anesthesia during surgery,” Warnaby said. “If we can prove further that this saturation relates to the point where people lose awareness of the outside world, it may change the way that anesthetics are delivered worldwide. Anesthesiologists would be able to give anesthetics to achieve this saturation level and know that they were giving each individual just the right amount of the drug.”

The research could also help resolve other riddles of the brain, Warnaby added. "Our findings could have implications for all sorts of altered states and disorders of consciousness, such as locked-in syndrome or persistent vegetative state."

In both the Oxford and U.S. research, scientists experimented with the common anesthetic, propofol.

There are EEG monitors available to assess the depth of anesthesia, although there isn't much evidence that these methods are better than traditional monitoring at reducing awareness during surgery, Warnaby said.

The next step is to perform further experiments to recreate a surgical setting. Researchers will look at how other drugs used during surgery—such as painkillers—affect slow waves during anesthesia.

“Depending on the operation, anesthesiologists have to give drugs that block muscle function, 'paralyzing drugs,'” Mhuircheartaigh said. “If inadequate anesthetic drugs are given while the patient can't move to let us know they're awake, awareness can occur.”

Like Warnaby, Mhuircheartaigh stressed the rarity of these cases, especially in healthy people. “However, rare isn't good enough,” she told Healthline. “We hope that by looking at this key process in the brain we can be sure that the patient can't perceive any surgery.”
